Preparing for Adulthood Transition Plan
The Preparing for Adulthood (PfA) - Young Person's Transition Plan is a multi-agency document to support young people with special educational needs and disabilities (SEND). The plan uses a person-centred approach to help the young people gather information ‘All About Me’, reflecting what is important to and for them as they make the transition from school to the next phase of their lives.
SfYP Advisers write a ‘Preparing for Adulthood’ (PfA) Transition Plan for young people from Year 9 onwards.
It is designed:
- to gather together robust evidence to help make decisions about the most appropriate option when they leave school.
- using the principles of person-centred planning to enable providers to support young people with additional needs to work towards their aspirations.
- to recognise that education is one part of a young person's life and allow scope for different agencies to take account of and plan for all aspects, creating a holistic package.
The PfA Plan is updated in subsequent years and includes the support needs of each young person. This information can be shared with colleges or training providers to help them plan for that transition. Copies of the PfA Plan are provided to the young person and their parent/carer.
Have your say on developments of our Preparing for Adulthood offer
We want to hear from young people with EHCPs, their parents/carers and professionals who work with young people about some proposed changes to the SfYP Preparing for Adulthood offer. Please read about the proposed changes and give us your views here: Preparing for Adulthood consultation. The consultation closes on 7 March 2025.
