My Futures

Supporting young people with special educational needs (SEND) joining, attending and leaving college

The Services for Young People 'My Futures' programme has been developed to support young people with SEND who need additional support to:

  •  Successfully transition from school to college 
  •  Sustain their place at college
  •  Transition into their next step after college, for example, further education, employment, training or volunteering

Young people with SEND may experience anxiety about change and unfamiliar places, and may need additional support to achieve their potential. Through the My Futures programme we work closely with schools and Hertfordshire's further education colleges to proactively identify young people who would benefit from additional support at an early stage as they transition to college, leave college or are at risk of ending their course early. We then provide tailored support as appropriate for each young person with an EHCP to help them develop the knowledge, skills, experiences and confidence they need to sustain their education, transition into employment or training, and ultimately make a smooth transition into adulthood.

Qualified Services for Young People Advisers and Youth Workers offer

  • Impartial one-to-one careers education, information, advice and guidance (CEIAG) at each stage
  • Suitable work experience and work tasters
  • Support to find volunteering opportunities
  • One-to-one support from a named Youth Worker
  • Referrals to youth work projects to help develop their confidence, social skills and independence if appropriate

Please use the Services for Young People referral form to refer a young person to this programme. The form can be completed by a professional working with the young person, a parent, carer or the young person themselves. 

If you are a young person with SEND in School Year 11, at college, or will be leaving college at the end of the academic year, please complete this short form and we will get in contact with you to discuss what help we can give you.
