Preparing for Adulthood Transition Plan

The Preparing for Adulthood (PfA) - Young Person's Transition Plan is a complete plan to support young people with special educational needs and disabilities (SEND) from Year 9 at school, in their transition onto their next step. It is designed:

  • to gather together robust evidence to help make decisions about the most appropriate option when they leave school.
  • using the principles of person-centred planning to enable providers to support young people with additional needs to work towards their aspirations.
  • to recognise that education is one part of a young person's life and allow scope for different agencies to take account of and plan for all aspects, creating a holistic package.


The Preparing for Adulthood Transition Plan is a multi-agency document and Services for Young People Personal Advisers have a key role in supporting the use of the plan.
